Postmortems for cost incidents

1. Concept

A postmortem is a written record of an incident, its root cause, and the action items that prevent recurrence. For cost incidents, postmortems follow the same structure as production incident postmortems with cost-specific content. The goal is organizational learning: making the team smarter so similar incidents are caught earlier or prevented entirely.

Without postmortems, anomalies recur. Same cluster spins up next month, same forgotten resource appears next quarter, same compromised credential pattern repeats. The postmortem is what breaks the loop.

ROOT CAUSE
─────────────────────────────────────────────────────────────────
A Kafka consumer was created on 2026-04-01 as part of a test
during the v3.2 migration. The cleanup script in the test ran
into a permission error and the cleanup logic was inside a try
block without retry, so it silently failed. The consumer was
left running with auto-tagging missing (the resource didn't have
a team tag), so it was never reviewed in the orphan-resource
audit. It ran for 45 days, ramping cost slowly as it processed
backed-up messages. The anomaly fired only after the cost rate
crossed the dollar threshold.

LESSONS LEARNED

1. "Untagged + high-cost" is a high-leverage signal that the
current tooling doesn't catch fast enough.
2. Test scripts that create real resources need cleanup
verification, not just cleanup attempts.
3. Slow-ramp anomalies are harder to detect than spikes; the
45-day window is bigger than our anomaly detector's typical
sensitivity window.
4. The orphan-resource audit needs to include untagged
resources, not just tagged-but-stale.

The compounding is the key benefit. One postmortem teaches one lesson; ten postmortems reveal patterns that drive structural changes.

What postmortems are NOT for

Blame assignment ("who screwed up")
Demoralization
Bureaucracy for its own sake
Performance reviews
Detailed forensic accounting

A blameless culture is essential. Postmortems focus on systems and processes, not individual actions. “Bob ran the cleanup script wrong” is bad; “the cleanup script didn’t surface its own failures” is good. The system that allowed Bob to make the mistake is what needs fixing.

When to write one

- Critical or emergency severity anomaly
- Repeated warning-level anomalies with same cause
- Significant org-level cost impact (>$5K, or >1% of monthly)
- Compliance-relevant events
- Anomalies revealing process gaps
- Anything an engineering leader asks for
DON'T WRITE A POSTMORTEM FOR:
- Routine warning-level anomalies
- Expected events with documented confirmation
- Anomalies resolved within 1 hour with no business impact
- Tests / dry-runs / planned exercises

Aim for 1-2 postmortems per month at a typical mid-size org. Fewer suggests anomalies are being closed without learning; more suggests the criteria are too loose.

The culture is mostly set by engineering leadership. If a PM identifies a process gap, leadership’s job is to thank the author and resource the fix, not interrogate why the gap existed.

If you can write this in 30 minutes, you have a postmortem habit. If it takes 4 hours, that’s normal for the first 2-3 you write; it gets faster with practice.
