Blameless culture
A blameless culture is essential. Postmortems focus on systems and processes, not individual actions. "Bob ran the cleanup script wrong" is bad; "the cleanup script didn't surface its own failures" is good. The system that allowed Bob to make the mistake is what needs fixing.
